Before the incident
Employee was packing product into boxes and pushing boxes into box taping machine.
What happened
When employee activated the box pusher to push box into machine the box jammed on edge of machine. Employee tried removing the jammed box without turning the controller key to move machine into home position and to turn air off to machine pusher. Air pressure was still present on pusher wo when employee unjammed the box the pusher moved forward. Employee lost balance and right hand and arm were pushed into machine jerking arm.
