Before the incident
Employee was retrieving a bucket (positioned on lowest shelf) off the storage rack within the Batch room to hand clean parts.
What happened
The bucket was stuck because a 50-pound scale calibration weight had been originally been placed on the shelf above it causing the shelf to slide down. The employee then took the calibration weight off and placed it on the bottom shelf. Then the employee attempted to move the shelf back to the original position with the use of an orange rubber mallet. This action caused the blade screen that is used for the Blending process and or rework tanks to fall off the storage rack striking the employees left shoulder then hit their left hand lacerating their pinky finger.
