Before the incident
At approximately [REDACTED] [REDACTED] a 4th hand on the 4 Paper Machine sustained an injury to his lower left leg while attempting to perform a front splice on a 265 diameter set after the winder had a hay out The injured 4th hand was immediately taken to Medical where he was seen by the onshift EMT Based on the employees initial evaluation by the EMT a call to the Jackson Parish Ambulance Service was needed for additional assistance and treatment After the arrival of the JPAS and evaluation it was determined that the employee required a next higher level of medical treatment and was transported to Ochsner LSU Medical Center in [REDACTED] Immediately following the incident safety standdowns were conducted with all crews and will continue to take place with all oncoming crews An incident investigation commenced immediately following the incident and a formal HOP Learning Event is scheduled on [REDACTED] [REDACTED] A shortterm corrective action has been developed and communicated to the
