On the above date and at approximately [REDACTED] company driver states as they opened the drivers side door to straight truck [REDACTED] they stepped back and slipped on some ice they did not see Driver immediately reported incident to their supervisor and stated their right ankle hurts a lot Supervisor observed driver limping while walking supervisor made the decision to have the driver examined by the Concentra doctor Upon a further medical examination it determined that our company driver sustained a right ankle sprain WASR and Transitional Duty Description are attached to this report Employee is feeling better and has been released from medical care work restrictions removed Please see attached updated WASR
