EMPLOYEE IS SUFFERING FROM PLANTARFASCIITIS THE EMPLOYEE WENT TO THE DOCTOR TO GET IT LOOKED AT BACK IN [REDACTED] AND A FEW TIMES SINCE [REDACTED] HE RECEIVED HIS TEST RESULTS BACK THEY STATED THAT THERE WAS A TARE IN THE MUSCLE OF HIS LEFT HEEL AND HE NEEDED TO HAVE IT SURGERY ON IT THE DOCTOR BELIEVES THIS WAS CAUSED BY THE HEAVY LIFTING AND GETTING IN AND OUT OF THE COMPANY BOX TRUCK
