EMPLOYEE STATED HE STOPPED AT ONE OF HIS STOPS [REDACTED] AND REACHED INTO THE VAN TO GET A LARGE HEAVY BOX HE SCOOTED A LITTLE FURTHER FROM THE EDGE ON HIS LEFT SIDE HE WAS TRYING TO BE QUICK BECAUSE OF A DOG OUTSIDE THE RESIDENCE WHEN HE PICKED UP THE BOX MOSTLY WITH HIS LEFT ARM HE FELT AN IMMEDIATE PAIN HE WAS STILL ABLE TO USE IT THE REST OF HIS DAY BUT IT WAS ACHING AND SWOLLEN UP AS DAY PROGRESSED HE ALSO STATED HIS SKIN WAS HOT TO THE TOUCH
