What happened
'Pipe was supported by two chain hoists and partially by one forklift fork. When the injured employee pulled the pipe flange towards himself the flange slipped off o the forklift fork and the pipe moved towards him quickly smashing his right thumb between two flanges.

The right thumb was smashed above the first joint in the thumb-nail area. The injury was diagnosed as having a bone chip fracture.

Two flanges on a 12 pipe came together trapping the injured thumb between them.
