IE was in the breakroom stepped off of the mat and slipped falling on their outstretched hand felt a pop in their shoulder unable to move R arm after the fall. Requested to be seen at a hospital. Transported in private vehicle with shift manager and EHS coordinator to local ER for further diagnosis and treatment.
