The employee had not started their shift. They parked their car and began walking to the [REDACTED] employee entrance to enter the facility.

The employee was walking to the West entrance by the timeclock before their shift started. They were a few feet away from reaching the door but the employee 's feet became entangled and they tripped fell forward. The employee had difficulty seeing in the darkness due to the motion sensor lights working intermittently.

The employee chipped their two upper-front teeth and bruised their upper lip. They also sprained their left knee and both wrists in addition to bruising to their right knee.
