Before the incident
On [REDACTED] at approx. [REDACTED] the EE was directed to pick up a serviced UPS pallet by the UPS lead in the DK6 service lanes. While attempting to pick up the pallet EE raised his forks too high and placed them against the front slat of the pallet. EE was unaware of his fork position and began to reverse into the pallet. Due to the forks being too high the pallet began to slide backwards. During this time the service operator had moved to the back side of the pallet to double check a labeling requirement. EE reversed and pushed the pallet into the service operator causing that EE to slide backwards and reach forward to brace on the pallet. The service operator who was contacted by the pallet was seen by ERT and reported left leg and hip pain. EE was given ice and cleared by ERT to remain at work. Injured EE also spoke with DK6 [REDACTED] and was to be placed within a different job function within regular duties. EE then elected to leave work due to the pain. EHS manager was notifie
