Before the incident

There was a roll change that happened. The employee went to take a film sample from the inboard roll and noticed a disruption to the running outboard roll

What happened

After the roll swung over the employee went to take a film sample from the inboard roll. While doing so the employee saw the outboard winder lost contact pressure causing the rolls to wind together. This caused the outboard winder to not keep up and they then determined the line needed to be shut down. When they turned to go to the control panel the right hand contacted a Plexiglas covered sign mounted on the side of the winder.
