Before the incident
employee was in the drive in cooler looking for bakery product and did not turn on the safety red light nor notify driver he was in the cooler He was in the middle of the cooler when driver started to put stacks of freight back into the cooler At that time again employee failed to tell him he was there until his shoe got caught under pallet As he pulled away to get out of the way he fell back on the floor Forklift driver stopped immediately check on employee and reported to manager
