What happened

On [REDACTED] Mr. [REDACTED] and Mr [REDACTED] were operating the machine at [REDACTED] local time. While Mr. [REDACTED] was operating the machine and Mr. [REDACTED] was observing him an adjustment was being performed on the upper portion of the press break tooling. The securement levers of the press break tooling had been loosened to reposition the upper portion of the press break tooling into the upper most travel of the securing fittings that are attached to the machine. After the securement fittings were loosened Operator [REDACTED] placed his hand in the opening of the machine and proceeded to lift up on the fixture. While lifting up on the fixture and preparing to retighten the securing fittings for the tooling Operator [REDACTED] repositioned his body to before retightening the securement fittings. While repositioning his body Operator [REDACTED] foot moved and landed on the cycle pedals which initiated the machine to run a cycle. The machine then ran the cycle and came down on
