Brief Description of Events: On [REDACTED] at approximately [REDACTED] Employee was selecting a staged case on the dock across from 220 door As he was getting his case and there was ice formed on the dock in that area from the ongoing humidity dock conditions [REDACTED] stated that he slipped on the ice causing him to do a split In doing so [REDACTED] strained his groin area [REDACTED] stated he did not fall to the ground I asked at the point was the ice visible he said no it must have been a thin layer of ice Also the dock had large fog on it causing visibility to be an issue as well He asked to take a little break and he went back to finish his stop Upon review the length of the dock had ice some spot had a thin layer others had more noticeable build up During and after this incident the dock personal was already aware and scrapping up ice I have the issue being monitored all day to help keep up with the issue
