Before the incident
On Extruder Winder #507 the roll did not transfer parallel onto the lowering arms after the cut over. The Lead Operator attempted to correct by using a hammer to force the roll to complete the cut over process. When attempt failed Lead Operator called for another operator on a forklift for assistance. Forklift Operator attempted to lift the film roll while EE attempted to pull the film roll forward. While EE attempted to pull the film roll forward the Forklift Operator lowered the forks causing the EE 's left ring finger to be caught between the Winder transfer forks and the film roll shaft.
