What happened
Teammate was working at the mixing deck performing a routine task cleaning the mixers. This task requires the teammate to safely follow and perform the AWP for this machine open the door to the mixer and clean the walls and legs of the mixer with a scraper. When cleaning the mixers the legs are required to be manually rotated with a handle (tool) to allow the teammate to clean all areas of the mixer. When cleaning the teammate did not use this handle (tool) designated for this task resulting in the teammates right middle finger being caught-in-between the side of the wall and the mixer leg.
