Moving a pallet of silane empty jugs from near the day tank area back to the raw material area
What happened
Teammate states that he was moving a pallet of silane empty jugs from near the day tank area back to the raw material area. Between the day tank area and just south of the B-side incline belt is a series of columns. The area teammate was working in is about a 20 ' x 30 ' area. Teammate had the pallet loaded onto the forks and was backing up south in order to turn around to head west. Teammate states he thought he was going maybe 5 to 7mph when he backed into one of the plant supporting columns. Teammate stated that when the incident occurred he thought he hit the back of his head on the top strobe light which is anchored to the forklift roll cage.
