A team of 3 was on a job site in [REDACTED] performing an installation of 2 5T overhead bridge cranes and runway structure Utilizing a spotter a flagger and an operator the team was moving a runway I beam into position using a telehandler The beam was lowered to ground level and sat down As the spotter was in front of the beam checking the position the operator began to reverse the telehandler before the flagger instructed raising the forks without realizing the tip of the right fork was still underneath the beam As the operator raised the forks the beam tipped and rolled forward landing on top of the spotters right foot just above the ankle causing scrapes bruising and swelling
