Injured EE asked another EE in the dept if he should cut the excess off the top of the box off and another employee in the department told him yes
What happened
EE was holding knife in right hand and holding top of box with left hand. EE started cutting the box horizontally towards left hand causing the knife to hit the forearm. EE was not wearing his required PPE ( Sleeves) and EE was not using the correct knife.
