Before the incident
EE lifted self dumping hopper on [REDACTED] and emptied the glass into scrap glass hopper
What happened
When EE brought hopper down the hopper head did not go back into place so the EE got off the PIT and walked to the side of the PIT and seen the hopper 's trip lever assembly was not lined up with the back pad assembly so he reached up to pull down on the trip lever steel handle but due to the pressure the handle smashed the EE 's thumb between the mast on the PIT and the trip lever.
