Mopping the Floor

On [REDACTED] around [REDACTED] I ([REDACTED]) was mopping the transit building by the rear door. I lost my balance (EE stated to me that his right foot got hung in the mat on the floor by the door) causing him to loose his balance. I lost my balance and fell to the floor while hitting my head arms elbows on the wall and my body landed on the hard floor. [REDACTED] (service attendant) found me laying on the floor and tried to assist me up but was unable to. [REDACTED] went and sought Ms. [REDACTED] (Transit Manager) who happened to be on site in the adjacent building. Ms. [REDACTED] was eventually able to help me up into a chair. Ms. [REDACTED] called EMS and they came and checked me out. I was then transported by private vehicle (by my brother) to Atrium Health Wake Forest Baptist Emergency Department-[REDACTED] to be checked out further from fall.
