What happened
[REDACTED] was adjusting a water valve underneath the cooling fan and his hand knocked off the fan cover. The fan cover clips were loose and the cover fell off leaving exposed blades. [REDACTED] inadvertently made contact with open fan blades and three fingers were lacerated by the blade.
