What happened
Two employees were assigned to work on line 45B to package leaf springs. Employees were having a conversation not paying attention and lifting leaf springs with one hand each. While doing this they pull slide the leaf spring off of a triple stack of unfinished leaf springs to which it lands on the right hand employee 's foot. Following up in the area the line leader didn 't enforce check PPE and the packagers didn 't wear it.
