Before the incident
Rolling Over Tub. An empty tub is placed on the floor and a filled tub is tilted over the empty tub so that the parts can be shifted into the empty tub. Process used if tub is overfilled or parts are placed into wrong tubs.
What happened
When tilting the full tub over the empty tub the employee did not notice the empty tub had slipped and lifted off the floor. When he moved close to start to shift the parts he slipped his foot below the lifted tub. when the tub started to fill it fell back down on his foot breaking a bone.
