Before the incident
On [REDACTED] employee was working in the acid tank farm alone when he was transporting a 200 lb. pump onto a cart. The grating in the tank farm that the employee was walking on is loose which caused the employee to lose his balance and drop the cart with the pump about a few inches below the grating onto the nitric acid return pipe. This resulted in the break of the nitric acid line where the acid was leaking from the bottom into the pan trench. The carts wheel was stuck in the grate and could not be lifted so the employee used force and tried to pull the cart out. The employee reported to be fine at the moment and the response team was dispatched to immediately shut off all machines suction the acid to clean-up the leak and then work to get the cart and pump off the pipe so the pipe can be repaired. The employee continued helping without pain and [REDACTED] reported to have tightness in the back. The EHS team asked if he wanted to seek medical attention to which the employee responde
