What happened
As he was jacking the pallet up the momentum of the weight caused the pallet to roll towards him quickly. Driver was unable to stop the pallet from rolling towards him so he pressed the pallet jacks release lever to lower the pallet. His right foot was not clear from being out from underneath the pallet and the pallet came down on his right foot.
