Walking into the facility to clock in to begin her shift.

The employee was coming into work and the exit door was propped open as the employee went to walk into the door her foot slipped under the rug and as she was falling her head hit the brick wall in front of her head and was unresponsive for 10 minutes and swelling of forehead abrasion.

Tripped and hit her head on the outside brick wall of the facility her head began to swell and she was airlifted to Trident Medical Center.
