Employee was changing a v-belt on a vibratory feeder at our [REDACTED].

What happened

Three (3) employees were changing v-belts on Feeder #8 on [REDACTED]2. The following precautions were taken: (1) Lockout Tagout performed and verified (2) Job Hazard Analysis was completed with an emphasis on pinch points (3) Tools were being used to keep hands away from moving parts (4) PPE being worn included hard hat safety glasses steel toe boots and gloves. One employee was positioned to manually turn the pulley shaft while another employee was positioned on a ladder to guide the v-belts into place. Another employee was in place to hold the ladder for the employee who was on the ladder. The employee on the ladder was guiding the belt onto the pulley using hand tools and instructed the other employee to manually turn the pulley shaft. While instructing the employee to turn the pulley shaft the employee guiding the belt onto the pulley got his glove against the belt pulled his hand in the pinch point and caused his finger to become wedged between the belt and pulley (tight working s
