Investigating a noise
What happened
Employee was under the Flatbread proofing bottom belt table 11 investigating a noise on the take up roller next to the scraper. He attempted push up on the belt to take tension off the roller to see if the noise would go away. At that point his glove entered the pinch point and pulled his left arm into the conveyor. He then opened the safety gate and stopped the line. When the line stopped the supervisor [REDACTED] went in to investigate the reason for the line stopping. [REDACTED] arrived and [REDACTED] the maintenance supervisor arrived [REDACTED]. [REDACTED] had to remove the tension roller and the scraper to get his hand out.
