What happened
Material being hauled to the dumpster by another forklift operator was inadvertently secured to the material rack itself. The injured employee attempted to help the other operator by cutting the material loose from the rack and in doing so caused the rack to fall several inches down onto their foot. Employee was not wearing required safety-toed shoes.
