GOP DOH [REDACTED] FAB 121005 [REDACTED] Hit rt leg First aid TM presented to hr treatment room at [REDACTED] TM job is chuck boner TM states on [REDACTED] on his lunch time he went to the mens locker room as he was walking to his locker the floor was slippery which caused him to slip he said he did not fall as he was able to grab on to the wall but his right leg crossed over his left leg and hit his right lower leg hamstring with the corner edge of the locker TM states he had mild pain yesterday did notify supervisor but did not get sent to safety he also said he came after work to Hr treatment room and safety office yesterday for pills but no one was in At home he put ice and also used hot water with salt to the area for relief [REDACTED] [REDACTED] TM came to hr treatment room requesting bio freeze because he was having pain on his leg due to the incident tm was advised we will have to do a report TM also mentioned he told his supervisor he was coming for pills only At this time tm
