What happened

The client had become agitated toward the end of lunch. He had been in the bathroom before leaving but sounded like he was choking so I knocked on the door to check on him. I didn 't get a response so I went into the bathroom and did a few back blows and the clinet was able to clear out what was causing the issue. Asked the client if he wanted some help cleaning up his coat and he was okay with that but had started to talk about wanting to hit someone. As we tried to leave the bathroom there was a small group of other individuals with disabilities that the client was a little too close to. I had tapped him on the shoulder which I had done before and was going to remind him we had to stay back a few more inches so it was probably partially my fault for startling him. He then yelled knocked my glasses off and caused a large scratch next to my left eye and a thinner longer scratch on my left cheek. After that I told the client it was time to head to the van.
