On [REDACTED] the Employee had been asked by their Supervisor ([REDACTED]). It was at this time that the Employee expressed they had pain in their wrist. The Employee stated that they had hurt their wrist the [REDACTED] but never reported it the Employee then went on to tell their leadership that they DID report it to their team lead and then the Employee recanted the statement saying once again that they did not inform anyone. The Employee opted to go to Madison Hospital under their own accord and use their own individual insurance coverage instead of informing Leadership Safety.
