Emptying a bin hopper with a forklift
What happened
An employee was dumping a small dump hopper of used knives from the grinding room into the large metal recycling bin with a forklift. The employee pulled on the chain attached to the latch holder to unlock the latch. After several attempts with the forklift raising it up backing up and going forward trying to get it to dump he then pushed up on the hopper with his hands to get it to move. Employee states that as he was pushing on the hopper he felt pain in his lower back.
