This nurse was told EE was standing on a ledge by a moving line in debone cleaning equipment. EE 's boot got caught in moving line and pulled EE 's right leg into moving line. Supervisor did note the EE was wearing his harness but the EE did not lock out tag out his machine before cleaning.
