Before the incident
A task of banding boxes to a pallet using nylon banding and a ratchet.
What happened
After looping the strap around the boxes the employee inserted the free end into the ratchet and began tightening the strap by moving the handle up and down turning the tension wheel. Once the strap was pulled tight around the load the employee used a separate gripper tool to clamp the strapping and hold the tension in place. With the strap secured by the external gripper he then used the built-in cutter on the ratchet to trim the excess strapping. At the moment of cutting the free end of the strap which was still partially wrapped around the tension wheel suddenly released upward hitting the employees ' right eye underneath his safety glasses.
