Employee had just started his shift prior to the incident. He was assigned a couple work orders one of which was to take care of a wire feeding into electrical that 's insulation had slipped out of the seal tite.

Employee found the wires undid the connectors for seal tite pushed insulation of wires into the electrical box pushed the rubber grommet up and that 's when the box arc flashed. The panel on the front of the box flew off and struck the employee in the head. Employee pushed wires into the electrical box without using a meter to check whether it was energized. Employee failed to de-energize equipment or even make attempt to prior to performing repairs.

Right eye and temple area impact causing blurred vision in the right eye w limited peripheral vision. Slight laceration and contusion above right eye at the temple. Brain fog. Temporarily loss of vision and hearing due to the arc blast.

Electrical Panel Door

Eye & Face Contusion along with vision impairment due to being struck in the face by the electrical panel door when bare wires made contact causing it to arc.
