Before the incident
Employee was attempting to rework a steel slit coil on the plant 's 24 slitting (repair rework) line. Several feet of the coil had to be scrapped due to damage and once removed the coil could no longer be held in place with the overarm 's snubber roll because a second coil (which was also on the uncoiler) now had a bigger O.D. than the coil being worked on.
What happened
A few outer wraps of the coil had unspooled so the employee began mechanically reversing the uncoiler in order to rewind the material back onto the coil. As they were jogging the uncoiler in reverse they placed their left hand fingers on the coil to help keep the steel aligned properly. The employee 's left middle ring and pinky fingers became pinched between the steel being rewrapped and the overarm 's snubber roll which was in the down position.
