Before the incident
IE and crew was in the process of offloading a pallet of densdek from a forklift to a cart. IE was providing hand signals to the forklift Operator. After placing the last pallet of dens deck onto the roof cart with a reach forklift two employees were trying to pull the roof cart away from the forks and were unable to do so.
What happened
IE then placed his right hand on the lift bracket behind the sitting position of the forks. The IE placed his right hand on the lift bracket to stabilize himself so that he could firmly plant one foot on the roof and to enable pushing the Dens deck with his other foot. As the material was offloaded the forks were freed up and dropped down to rest against the lift bracket. The tip of IE right ring finger was crushed in between the forks and the lift bracket.
