Employee and 2 others were setting up a fixture to encapsulate a tank in order to prevent expansion when putting under water for leak test.
What happened
When the two other employees were helping lift the clamp of the fixture the weight and awkwardness caused the one employee to lose grip. The effected employee on the other side had his hand in the way. The top of the fixture came down and smashed EE 's right hand.
