What happened
EE states that he told supervisors back in [REDACTED] that his left elbow was sore. EE later told EHS in [REDACTED] that he had some soreness in left elbow. When asked how it happened the EE couldn 't pinpoint a specific task. EHS asked EE if he wanted to have it checked medically. He stated he would wait and see how it felt. On [REDACTED] the EE gave EHS a medical form stating a diagnosis of Bicep tendinitis. There are no restrictions on the medical form
