Before the incident
The employee was loading component racks in the back of a box truck with a forklift.
What happened
Forklift became stuck inside box truck. Employee exited the forklift and entered the box truck to investigate. The employee discovered forklift tines were on top of rack one and contacting the floor of the truck. The employee pushed rack one away from the forklift tines. This released the energy then one of the forks struck the left foot of the employee.
