The employees written documentation of the incident is as follows. Fully describe what happened in sequence listing time as accurately as possible. While lifting a very large man greater than 300#. I placed the stretcher in the back of the ambulance so the front would latch. [REDACTED] was assisting in lift assist. 2 firemen and my partner started lifting the stretcher before I was ready 1 fireman stopped lifting and moved away from the stretcher in the middle of the lift leaving only one fireman and my partner to lift to stretcher. This left the stretcher lopsided so I grabbed onto the stretcher to help lift into the back of ambulance 108. In doing so I strained my lower left side of my back.
