What happened
The employee was loading a OTR truck that was backed down to the dock when he noticed some pallets that were leaning. The employee went back inside the OTR with his forklift to correct the leaning pallets. The dock clerk signaled to the OTR driver that he was good to pull out unaware that the forklift operator had went back in to fix the pallets. When the OTR driver pulled away from the dock employee still on his forklift rolled out of the trailer and of the truck hitting the ground.
