Before the incident

Employee was preforming PM activity on the conveyor sections inside the point of operation of the tempering furnace load end lift conveyor. Employee had a grease gun with him at the time of incident; therefore it is assumed he was applying grease to fittings at the time however no assigned PM work was scheduled for this piece of equipment at the time of the incident.

What happened

Employee was preforming PM activity on the conveyor sections inside the point of operation of the lift conveyor transfer. Conveyor equipment cycled while PM Tech was inside the point of operation and the lift conveyor pinched- trapped the employee between the lift conveyor and the upper transfer conveyor. Operator was pinned at the chest level between his back and chest (back facing upper transfer chest facing lift conveyor.)

Injury or illness

Chest and Back was pinched trapped between the Upper transfer conveyor and the lift conveyor. Chest toward the lift conveyor and back against upper transfer conveyor.

Object or substance involved

Lift conveyor and Upper transfer conveyor trapped pinched employee upon automatically cycling. It is assumed the employee tripped a glass sensor causing the machine to cycle (equipment not locked out from energy sources.)

Summary line

Employee was preforming PM activity on the conveyor sections inside the point of operation of the lift conveyor transfer. The Conveyor equipment cycled and pinched-trapped employee between lift conveyor and upper transfer conveyor.
