Claimed lifting bowed glass

A Team Member TM was absent from work on [REDACTED] and [REDACTED]. He stated he had back pain but there was no indication that this was work-related as he had a wedding on [REDACTED] where he did some bowling and dancing. He was working with Human Resources and was about to receive FMLA paperwork when the TM said it was work-related because the Doctor note indicated it was with a DOI of [REDACTED] 6 [REDACTED]. At no point prior to this was the TMs lead supervisor or any member of management made aware of an injury. The TM said he was lifting bowed units and felt a tweak in his back. He stated via phone call he has felt it before and was fine but this seemed different. We checked the RHEO cameras and the bowed units were ran on [REDACTED]. There is no indication that the TM had a work related back injury. TMs lead and supervisor were told by other co-workers that this was a non-work related injury.
