What happened
Safety Bar and Photo eye on one of the pegs was being made and nothing was present on the pegs. Then two employees leaned over not breaking the area safety scanner to remove debris from the eye on the pegs when the downstream eye was cleaned off the unit came out of the sealer hitting the other operator who just reached in. Unit on the shuttle came in contact with his right pointer finger.
