4 7 Incident: The Employee had finished a rack on destination 4. As he stepped to the side he felt a tightening on the lower bottom left side of the back for only a few seconds. The Employee informed the Shift Manager that his back was feeling tight and was going to visit the clinic. The employee did not say it was due to a work event. The clinic nurse instructed the employee to use ice heat and take over the counter [REDACTED]. 5 10 Update: The Employee visited the clinic for their annual physical. While there they complained of back tightness. The nurse was not aware that this was in connection with the incident on 4 7. The nurse prescribed a 6-day supply of steroids and muscle relaxants. The documentation from this visit was placed in the employee s personal medical records. 5 31 Update: The employee came back with complaints of back pain and the clinic recommended a referral to an outside specialist.
