Preparing the next order of glass to be run and making active adjustment to the machine.

The operator was preparing to change the size of the glass running through the [REDACTED] Grinder from 3.0mm thick 29 1 2 x 30 7 8 to 21 31 32 x 63 31 32. The operator failed to de-energize the [REDACTED] edger machine before entering the conveyor section where adjustments are performed. Upon entering the machine the operator stepped on an active conveyor belt which in turn pulled his left foot towards the input section with stationary guarding with low entry. This caused the foot to be pinched and crushed. At this point the operator began yelling for help from his supervisor who immediately rushed over to reverse the conveyor and shut down the machine along with having a team member call CODE BLUE Medical Emergency on the in-house intercom. Once the operator s foot was removed from the machine he was taken to the internal first aid room. The in-house first responders noticed that the injury was beyond first aid and called [REDACTED].
