Dumping broken cullet from inspected lite
What happened
The incident occurred on [REDACTED]. The employee works as a furnace operator on Furnace 3. In the evening on the [REDACTED] he was doing a quality test of our tempered product. This is done at a small dump table made specifically for the test. The table is hinged and balanced so that the sample lite can be easily dumped into the cullet bin by lifting the table end. The injured employee was standing on the other side of the dump table from the pictured employee during the dump. The employee lifted the table to dump it and when the table cam back down the employee his the top of his forearm just above the wrist on the corner of the test table that is butted up against the dump table. The employee first reported this incident as a first aid to his supervisor later that shift the [REDACTED]. The employee reported through the week that he was experiencing some pain at the impact point but that he didn t want to seek further care. On [REDACTED] he reported the incident to the safety manager
